This group is regarding the Kentucky Medicaid Program. I specialize mainly in the Home & Community Waiver. Under the Home and Community Based Waiver, there is little to no cost to client for Home Health Care, Adult Day Care, (a mixture of both) and Case Management Services( (monthly check-ins, and they order monthly, if any supplies are need.) Once I receive a referral I immediately do the application; calling the client if further information is needed. My goal is to do the whole application process with the member having to do nothing on their part, but schedule assessment & do Level of Care assessment with the State Nurse Assessor. There are so many in our community that can benefit from this program!
